Send MIDI-CC (Controller)

Hi,
is it possible to send a MIDI-CC to a) an external Synthesizer b) to a VSTi?
E.g. for automation, send CC#74 (Cutoff)
I found it is possible to connect (hardware) knobs via a CC to LMMS knobs,
with rightclick, 'connect to controller' but i need it the other way round.
